Sit with the users. Watch the workaround. Every design decision traces to something a real person did, said, or couldn't do.

Prototype

Make the idea clickable before making it expensive, flows tested on paper-cheap prototypes while change still costs nothing.

Usability sessions with the actual audience, task success, error recovery, and time-on-task measured against the release before it ships.

508 and WCAG 2.1 AA validated as part of the definition of done, accessibility is a gate the release passes, not a memo it receives.

The loop

Findings get fixed, not filed.

On a DHS mission program, HCD research and usability testing run inside the DevSecOps teams, a finding raised in a session becomes a ticket in the same sprint, and the Tier 2/3 service desk closes the loop from the field.
